This Supervisor for Adult F oster Care supervises the Adult Foster Care (AFC) program within Community Services, offering support, training, and coaching to caregivers and program members . The AFC program provides a home-based care setting for individuals who need assistance with daily living activities, and the supervisor plays a key role in ensuring the quality and effectiveness of the care provided.

Essential Functions

• Provide direct support to the care managers/nurses in the community.
• Conduct site visits to consumer homes and produce proper quality feedback /reports according to manager’s guidance.
• Offer input to annual performance evaluations.
• Provide monitoring and oversight for efficiencies with all referrals in AFC.
• Lead and assist with program team meeting.
• Support and train staff during the orientation processes or initial planning meeting to insure proper efficiencies in services.
• Support Community Living Program Manager as assigned.
• Monitor LTSS process to ensure accuracy and timely submittals.
• Assign caseloads.
• Provide supervision to nurses and care managers.
• Handle member or caregiver complaints.
• Support new program development and implementation with program.
• Additional responsibilities as assigned.

Competencies

• Three or more years of recent experience working with people who are elderly or disabled.
• Ability to supervise and direct staff
• Excellent organizational skills
• Attention to accuracy and detail
• Strong interpersonal skills – telephone skills
• Excellent oral and written communication
• Good computer and analytical skills
• Innovative thinking
• Ability to maintain confidentiality
• Perform all other duties as may be required

Preferred Experience

• Supervisory experience preferred.

Required Education

• Bachelor’s degree in Human Services or related field

Work Environment

• This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
